In line with our Data Protection policy, current legislation and our commitment to quality on line post adoption services for the Adoption Community this form has been posted to allow all people with an interest in adoption, or any other, to check if the North East Post Adoption Service holds information about themselves.

This practice is in line with the Data Protection Act 1998.

Procedure:

  • The Subject Access request must be made on the recognised NEPAS request form, down loaded and forwarded to us or by submitting on line.
  • Additional information if requested must be given to help NEPAS meet your request.
  • The necessary administration charge must be forwarded with the request.
  • A valid address and email address (if available) must be forwarded with the subject data request form.
